Additional commodity futures risk disclosure (only if your meet the requirements).

Request to receive your commodity futures statements by e-mail.

Copy of a government issued I.D.

Appropriate W-8 forms (needed if you are not a legally recognized citizen or organized entity within the U.S.).

Any of the other additional commodity futures trading documents that may be required for special situations.
